#1  W9 Form 

A W9 Form is required to conduct payroll. Please complete the W9 Form below by clicking the link.

SSN or EIN number is required to be on your W9 to be accepted. Failure to not complete the document in full will result in your delay or decline to join the Association. 

#2 SHO Independent Contractor Form 

The Independent Contractor Form serves two key purposes for Seacoast Hockey Officials:

1. Expectation Agreement: By signing the form, the candidate acknowledges and agrees to the expectations set forth by the Association to be a member. This ensures all officials understand their roles, responsibilities, and conduct standards.

2. Operational Transparency: The form outlines how the Association operates, providing members with clarity on policies, procedures, and the overall structure. This helps officials understand how assignments, payments, and other operational aspects function within the organization.

(3) Parental Agreement Form 

Only required when signing up a minor. (Under 18 years old)

This agreement is required for any minor (under the age of 18) who wishes to work for Seacoast Hockey Officials (SHO). A parent or legal guardian must review and sign this agreement before the minor can be approved to work.
